I don’t want to be the party-pooper… but someone has to say it.
Aligners have changed dentistry in amazing ways. I use them, I recommend them.
But they also come with a plastic problem we don’t really talk about.

Every treatment = dozens of plastic trays.
Multiply that by thousands of patients… and the numbers get wild, fast.
Recycling exists (hello TerraCycle 👋♻️) but barely any practices offer it and even fewer tell their patients about it.
So most aligners? Straight in the bin.

Meanwhile we’re all trying to live more sustainably… except in dentistry, where we sometimes forget that medical products also leave a footprint.

Adult Open Bite Case, Age 36
This 36-year-old patient presented with an anterior open bite, leading to difficulties with incising food and contributing to increased strain on the posterior teeth.

Treatment focuses on correcting the vertical discrepancy, improving anterior guidance, and distributing bite forces more evenly. Depending on the case, approaches may include fixed appliances, elastics, or skeletal anchorage.

Adult open bite correction is possible with the right diagnosis and biomechanics.

I still remember my first deep conversation about feminism with my partner. He’s a cultured, empathetic and genuinely well-informed man. Yet even for him, some of the core principles were hard to fully grasp.

I remember thinking: if there’s one man in the world I want to understand what this really means for women in daily life, it’s him.

Sometimes patients ask me why treatments can feel long and expensive.
It reminds me of a story my tattoo artist told me when I first sat in his chair over 10 years ago:

Why I Got a Second Phone for Work 📱📱

2 years ago I decided to get a second phone dedicated only for work. It’s been a game-changer, and here’s why:

🔹 Clear separation between life & work
On my work phone, I get messages from managers, nurses, labs, reports, X-rays (sometimes other people’s teeth 🦷). On my personal phone, I get my mom’s codfish recipes and my dad’s fishing victory photos 🎣😂. Mixing them all was messy and overwhelming.

🔹 Boundaries = less stress
I set fixed times to check my work phone: 11:00, 15:00, and 18:00.
During those windows, I reply, handle pending matters, and get things sorted. Outside of that, I’m not glued to constant notifications.

🔹 Freedom to disconnect
If I decide to completely unplug, I can literally leave the work phone behind. That way, I’m not tempted to “just check quickly” and suddenly spend an hour solving work problems.

🔹 Better focus
Work chats don’t interrupt family time, and family group messages don’t distract me during professional tasks. Both worlds stay organized and respected.

🔹 Respect from colleagues
By not being always-online, people understand I’m not instantly available. It creates healthier expectations while still keeping communication structured.

In short: one phone keeps me human, the other keeps me professional.

If your job is full of messages, photos, and documents like mine, I highly recommend trying this setup. It’s a small investment that pays off big in peace of mind.

As both a dentist and a mother, I pay close attention to the daily habits that influence my child’s jaw development, breathing, and long-term oral health. A few key principles I follow:
• 🍼 Breastfeeding when possible – encourages natural jaw and muscle development.
• 🥦 Offering whole foods – chewing a variety of textures strengthens the jaws and stimulates growth.
• 🥤 Avoiding sippy cups – instead, we use an open cup, which supports correct tongue posture and oral function.
• 📖 Encouraging singing, reading, and speaking aloud – promotes orofacial muscle development and healthy breathing patterns.
• 🌳 Plenty of outdoor activities – nasal breathing, good posture, and physical activity all contribute to airway health.
• 🌙 Monitoring sleep – watching for signs such as snoring or restless sleep helps detect possible airway concerns early.

That’s why it’s so important for children (ideally around age 7) to see a dentist who evaluates not only the teeth, but also the oral and nasal airways. It’s about more than smiles, it’s about whole-body health.

While my role focuses on identifying and correcting developmental issues as early as possible, I also recognize the tendency in today’s society to over-treat based on aesthetic expectations. This trend often extends to children. It is important to understand that, in the absence of functional problems and when adequate space is available, features such as mild crowding, small rotations, or midline spacing are frequently part of normal development and may resolve naturally or be more appropriately addressed at a later stage. Careful assessment helps ensure that treatment is both necessary and beneficial.

When a baby tooth still plays a role in an adult smile

This patient came in with a missing lower permanent incisor (congenital agenesis). A primary tooth was still in place, and although its long-term future is uncertain, we chose to keep it as part of the treatment plan.

The patient fully understood the possibilities, including that the baby tooth may eventually be lost but together we decided it was worth keeping for now to maintain both function and esthetics.

At this stage, the tooth is stable. The approach: monitor carefully, adjust if needed, and aim for the best balance between biology, function, and a confident smile.

I know, this is always controversial among my colleagues. But save the hate through your DM’s. It is what it is. And bruxism is not only about enamel. In fact, enamel damage is just one of the consequences and usually not the most impactful one.

The truth is, bruxism is rarely an isolated problem.

It’s a symptom, not the disease.
• Grinding is strongly linked to micro-arousals in sleep, airway resistance, and even sleep apnea.
• It’s associated with tension headaches, jaw pain, neck pain, and damage to dental work.
• It fragments sleep quality, which means people wake up exhausted, anxious, and less able to function during the day.

If we stop at “wear a guard,” we’re only protecting teeth while ignoring the fact that:
• The jaw muscles, TMJ, and even the airway may still be under constant strain.
• Sleep may still be poor, affecting memory, mood, heart health, and overall quality of life.
• The underlying triggers — whether stress, medication, or airway obstruction — go unaddressed and can worsen over time.

That’s why a proper sleep-focused evaluation makes sense. It allows us to:
• Confirm if grinding is linked to sleep apnea or other sleep disorders.
• Identify whether the issue is stress-driven, neurologic, or respiratory.
• Tailor treatment beyond just “mouth protection,” so the whole patient benefits.

The retainer had broken and shifted, and instead of holding the tooth safely in place, it slowly guided it into a harmful position. Over time, the bone support around the tooth was lost, and by the time it was checked, the tooth was so mobile that it couldn’t be saved.

It’s a reminder that orthodontics doesn’t simply “end” when braces or aligners are removed. Retainers can break, they can move, and without follow-ups, they might do more harm than good.

Regular reviews and a little vigilance can make all the difference in protecting your smile for life.

Who Am I trying to impress?

I’m not trying to impress my child self with my career achievements. I think children should be left alone and we should all stop asking them what they want to become when they grow up. They will figure it out and the last thing we want is children having concepts of professional success.

On the other hand, that was me in 2015, sitting on my wooden floor of my very (very) old, rented flat in Lisbon.
And at this point of my life I was just paying my rent and gradually getting furniture to decor the flat. All I had at this stage was a sofa(freshly bought!) a bed, a fridge and a dog.
And this was the case because I was funding my pos-grad in Orthodontics.
And this might be silly for many of you readers but let me enlighten you: a young dentist reality, back in Lisbon, 10 years ago was harsh. At least my story was.

Frequently being asked to stay at the practice over non paid hours, blackmailed to sign abusive work contracts, as a young woman being hyper sexualised at my own job from colleagues and chiefs, working with very low paid dental insurances (I remember there was one insurance paying 1€ for dental cleanings).
And her, the girl on the left, she went through that and with no much than a dog, a fridge and a bed, she made her journey through orthodontics. ✨

I’m not trying to impress my child self with my career achievements. I think children should be left alone and we should all stop asking them what they want to become when they grow up. They will figure it out and the last thing we want is children having concepts of professional success.

On the other hand, that was me in 2015, sitting on my wooden floor of my very (very) old, rented flat in Lisbon.
And at this point of my life I was just paying my rent and gradually getting furniture to decor the flat. All I had at this stage was a sofa(freshly bought!) a bed, a fridge and a dog.
And this was the case because I was funding my pos-grad in Orthodontics.
And this might be silly for many of you readers but let me enlighten you: a young dentist reality, back in Lisbon, 10 years ago was harsh. At least my story was.

Frequently being asked to stay at the practice over non paid hours, blackmailed to sign abusive work contracts, as a young woman being hyper sexualised at my own job from colleagues and chiefs, working with very low paid dental insurances (I remember there was one insurance paying 1€ for dental cleanings).
And her, the girl on the left, she went through that and with no much than a dog, a fridge and a bed, she made her journey through orthodontics. ✨

We’ve created space and are now in the alignment phase, steady progress while keeping tissue health and root integrity in mind.
